The information required to file personal and household income tax is as follows: the names of the whole family, social security number (also known as "work card" number), date of birth, occupation, family address and telephone number; salary forms, such as W-2, 1099-Misc, 1099-G (unemployment benefits), 1099-R (pension), social security benefits statement, gambling income statement, etc.; bank interest statement (1099-INT), dividend statement (1099-DIV) and stock statement (1099-B). If there is no 1099-B, the transaction dates, prices and records of profits and losses of the stocks and mutual funds must be listed.
If you have your own business or have bought or sold houses, you need to provide sales contract and closing statement, mortgage interest statement and real estate tax statement. If you have rental, you need to provide proofs of rental revenue and expenditure. Expenditure items include expenses of insurance, utilities, maintenance, etc. If you are a part-time worker or a freelancer, you should provide proofs of income and expenses. Stockholders of partnerships and small corporations need to provide income statements (K-1). Proofs of purchase of health savings accounts, individual retirement account (IRA) funds, student loan interest, moving fees, divorce alimony and bank fines should also be provided. Records and receipts of work-related expenses should also be provided, such as professional conference registration fees, air tickets, hotel fees, the purchase of computers, printers and other receipts, book fees, professional training tuition receipts, etc. If you have made donations to charities, you also need to provide receipts.
